Indicated by medical personnel after audiometric evaluations and reports of hearing loss after treatment, obtained by attending physicians in the process of anamnesis;

Platelet count less than 100,000 per microliter of peripheral blood

Red man syndrome

Indicated by attending physicians and reviewers, faced with the following standard of diagnosis: flushing, itching, chest pain, muscle spasm or hypotension during infusion of vancomycin
